The sender’s last name is noted as MATSCHIZKO.
Could it be that the recipient’s name is MATSCHIZKO, Micha?
If yes, what do the appendices mean (marked yellow)?

„Iba Lager Gaustadt“ most likely refers to „IBA / Interessengemeinschaft Bamberger Arbeitslager“ (Bamberg Labor Camp Interest Group)
In Gaustadt (today a city district of Bamberg) there was a labour camp of the spinning mill ERBA.

I think its an attempt to decline the surname. So its like to whom is this letter addressed to.
Would also agree on the name Micha ( Михайло - Михась - Михасю) same with his given name its probably also declined here.

Yes, they are making a ukrainian “Dativ” ot of names in “Nominativ”, and then transliterating the result into latin script. I have seen this in many cards. It is still ok to fill it out as written, or if you can recognise this - to duplicate fields and make corrections - or just skip the part with the receivers data - we will sort this out later, we know that the receivers names and adresses are filled out especially chaotically and would require a lot of attention in the processing stage.
