w.o. = wie oben / as above. It refers to the place of birth mentioned „above“ in the card, meaning the place of birth and the place of residence are identical. So please enter „Roccacasale“ in POB and also in "relative’s residence " - (Some questions + answers, terms etc can also be found here – just in case you haven’t found this yet!)
As for your other question: I probably would enter „unclear“ here as well.
In general it’s better to attach questions like this to the corresponding document, so it’s clear what exactly it is about.
And you can always add additional info in your comments of a specific document (here for example a complete name you may have found in other sources.) (it’s optional of course, not a Must!)
