Was that correct? (in his other cards he is the „sender“)

Yes, its correct to write it down this way, while we have to depict the exact information that we see in the card.

I can assume that this form was also filled in by him (Iwan), as the typing errors and handwriting match. However, as I mentioned earlier, we cannot include such assumptions in the form and should leave it as you have done.
