This document is a little unusual from what I have seen, not sure what exactly is going on

Hi, there are different types of Bremen cards. This is an early one and less frequent than the others. It was used from April 1946 onwards for the DPs who were able to emigrate to the United States on the first ships.
See here, (explanation in German…) and here (the e-guide) Both of them are also linked in the introductory text to the cards.

Sometimes some information is missing in these cards. In this case there are no details of departure (destination, date, ship) So please fill in what is given (name, date of birth, nationality, religion) and leave the other fields blank.
